The error lies in the " are made " part of the sentence. Not able to guess what type of the error it is. But it sounds different / unnatural. It ought to be "should be made" or the like.

In B, there are two thats,

It seems that the sentence would not sound good w/o a that in it. So, I guess C would is the correct option.

The answer is B

It uses the Idiom requires that X be Y correctly.

By the way what is OA ?
